PHARMACOLOGY
Pharmacotherapeutic Group:Beta lactam antibiotic
Pharmacodynamic Properties:Cefixime is third generation oral cephalosporin with broad activity against gram-negative bacteria. It has bactericidal action. It binds to one or more of the penicillin-binding proteins (PBPs) which inhibits the final transpeptidation step of peptidoglycan synthesis in bacterial cell wall by interfering with transpeptidase enzyme, thus inhibiting biosynthesis and arresting cell wall assembly resulting in bacterial cell death.
Pharmacokinetic Properties:
Absorption:About 40%-50% absorbed orally
Distribution: Serum protein binding is concentration independent with a bound fraction of approximately 65%.
Metabolism: Not metabolised significantly
Excretion: Mainly via the urine, and small amounts eliminated in bile.
INDICATIONS
Uncomplicated UTI, Uncomplicated Gonorrhoea, Typhoid fever, Acute Bronchitis
DOSAGE & ADMINISTRATION
Adult: 200mg twice daily or400 mg as a single dose. Child (6 month or older): 8 mg/kg/day administered as a single dose or in two divided doses.
ADVERSE DRUG REACTIONS
Diarrhoea, nausea, vomiting, abdominal pain, headache, dizziness, thrombocytopenia, eosinophilia.
Potentially Fatal: Pseudomembranous colitis.
PRECAUTIONS:
History of allergy to penicillins, pregnancy, lactation, renal failure, gastrointestinal disease
DRUG INTERACTIONS
Increased concentrations with probenecid. Increase in plasma levels of carbamazepine when administered concomitantly.
Potentially Fatal: May increase prothrombin time with anticoagulants
CONTRAINDICATIONS
Known allergy to cefixime or other cephalosporins.
