Pharmacotherapeutic Group: Anti-Hypertensive (Angiotensin receptor blocker (Angiotensin II receptor antagonists) with many properties similar to ACE inhibitors.)
Pharmacokinetic properties: Angiotensin receptor blocker has many properties similar to ACE inhibitors. However, unlike ACE inhibitors they donot inhibit the breakdown of bradykinin and other kinin and are less likely to cause the persistent dry cough. They are therefore a useful alternative to patient who have to discontinue an ACE inhibitor because of persistent cough. Absorption: Rapidly absorbed. Dose dependent bioavailability 42% (after 40 mg dose); 58% (after 160-mg dose). Peak plasma concentrations: 0.5-1hr.Distribution: Protien binding: 99%. Metabolism: Undergoes conjugation with glucuronic acid to form inactive metabolites. Excretion: Excreted almost entirely as unchanged drug in faeces via bile. <1% excreted via urine. Terminal elimination half-life: 24 hr.
INDICATIONS : Hypertension, Heart Failure, Myocardial Infarction, Prevents complications in CVS and Diabetes
DOSE : Hypertension: 40 mg once daily. Dose can be increased to a maximum of 80 mg once daily. Cardiovascular prevention: The recommended dose is 80 mg once daily. Elderly patients: No dose adjustment is necessary for elderly patients

SPECIAL PRECAUTIONS Hepatic insufficiency, biliary obstruction, renal impairment, renal artery stenosis. Correct volume depletion before initiating treatment. Monitor serum potassium level regularly in elderly patient and renally impaired patient.
ADVERSE DRUG REACTIONS : URTI, dizziness, back pain, sinusitis, pharyngitis and diarrhoea. Slight elevations in liver enzymes. Potentially Fatal: Rarely angioedema, rash, pruritus and urticaria.
DRUG INTERACTIONS : Concurrent use increases digoxin concentration; increases risk of lithium toxicity; increases risk of hyperkalaemia with potassium sparing diuretics, heparin. PPRESENTATION
